Step 2. Schedule a Lifestyle Medicine Initial Consultation

Step 3. After your first visit, you can continue to your Lifestyle Medicine follow-up sessions.

Services Include:

1. Lifestyle Medicine 60-minute Initial Consultation
   Ready to make a change or just want to start feeling better? Let’s work together to help you become the best version of you. Take control of your health now, the lifestyle medicine way! At this visit, you can expect:

• review of medical history, labs and lifestyle questionnaire
• vitals
• body composition analysis
• lifestyle goal setting
• development of action plans
• wellness counseling and coaching

For Cash-Pay (Non-Medicare/Medicaid) Patients: $199

For Traditional Medicare Patients: Billed to Medicare. Coverage subject to your plan’s benefits.
Included Services: Same as above, but each item will be billed through your plan according to its coverage rules.

 

2.  PREMIUM Membership ($179 per month)

Note: Patients with Medicare cannot enroll in memberships—those plans require individual billing per visit or service. 

May select after initial consultation. By complementing the care of your primary care physician or your specialist, Dr. Onyx MD PhD can help you achieve your health goal by focusing on lifestyle changes. Whether it’s to manage or prevent a chronic condition, as a member, you have your doctor counseling and coaching you throughout your health journey.
- review of medical and lifestyle medicine assessment
- two 30-minute visits (virtual or in-person) per month; each additional visit will be a $99 fee
- access to the doctor for non-urgent issues in between visits via email, phone call or text
- up to 2 body composition analysis sessions per month
- lifestyle medicine coaching and counseling with Dr Onyx
- prescription for medications as needed
- lab and test orders as needed
referrals to other specialty services as needed

 
3.  CORE Membership ($99 per month) 

Available to a select number of patients to help maintain personalized care. By complementing the care of your primary care physician or your specialist, Dr Onyx can help you achieve your health goal by focusing on lifestyle changes. Whether it’s to manage or prevent a chronic condition, as a member, Dr Onyx counsels and coaches you through your health journey.

4. PAY PER SERVICE

Lifestyle Medicine 30-minute Follow-Up $129

You’ve made your first step with the initial Lifestyle Medicine Consultation but would like more guidance on your health journey without being a member. At this visit, you will discuss labs that have been ordered or review action plans from your initial consultation. Your last appointment with Dr Onyx must be within 3 months to be considered a follow-up visit.

Body Composition Analysis 15-minute Visit $49

Whether you’re on your weight loss journey, reaching for a higher level of fitness, or simply want a glimpse of your health, your fat, muscle, and water levels measured by body composition analysis will help us better assess your health risk and monitor your changes. To accurately track your progress and be confident you’re heading the right direction, it is recommended that you check your body composition every 2-4 weeks. 

 

IMPORTANT BILLING NOTES
Traditional Medicare/Medicaid Patients: Cannot enroll in memberships (Premium or CORE). All services must be billed individually to your coverage plan in compliance with federal guidelines.

Commercial Insurance Patients: Memberships or services are not reimbursed by commercial insurance.
If you enroll in a membership or single visits, fees must be paid out-of-pocket, but we can give you a superbill to submit to your insurance.

Cash-Pay (Non-Medicare/Medicaid) Patients: All services (including initial consultations, follow-ups, and specialty services) are paid out-of-pocket.
Membership fees are also out-of-pocket and cannot be billed to any insurer.

Lab and medication are billed separately.
